I was told that when someone in the UK is diagnosed with a serious but treatable medical illness, they are now given a treatment options document rating hospitals on survival rate, cleanliness, privacy etc.

I am looking for an example of one of these sheets and any help would be appreciated. I couldn't find one after some time spent on google.
 
i recall Mantra posting in a US/Canada health care type thread, i guess he studies it or something...

however, aside from the possibility of contracting myself a serious but treatable illness, i'm not sure if i will get the opportunity to snag one of these forms....

I assume it will be local hospitals, so will be based on postal code. There is an online version that does much the same thing but I don't know if that's what you want.

I've never seen one of these forms, and like coombz says, short of contracting something myself its unlikely I will!
