Then the next attempt, I only had 90% of his threat and pulled aggro off the MT (and was 2-shotted).

KTM Underreports hp/mp restore.

Hateful bolts are not aggro based in that sense. They hit whoever is the highest hitpoints in the raid, that is not the main tank.

They hit second on his threat list.

Thats what we had always assumed as well. We had a fury warrior in AR gear OTing him and eating bolts. However, at times random people would get bolts. The bolts definitely hit the highest-hp nonMT. Try it We just have a warlock load up stamina gear, and keep himself up with drain life etc, now.

Edit: Should mention that anytime my hitpoints were higher than the warlock's I got bolted because I was next highest.

My raid leader is also of the belief it's 2nd highest HP. I'm too busy during the fight to notice.

I talked with our guilds top lock today and he confirmed that. Now I can take salvation and not worry about dying. (yay)
