The creepy thing is, yes.

My granfather died of Alzheimer's. One day, in the later stages when he was barely recognizing anyone anymore, he and his wife were sitting around at home, when out of the blue he turned to her and just said "but at least I'm still alive". Poor woman was beside herself for days.

So yes, people with dementia are well aware of the fact.
