in the US an hour typically means one service unless you negotiate differently beforehand. in canada with the known quality agencies an hour means an hour. if you can bust 10 times in that hour you'll be serviced 10 times. and no, you always pay the full hour even if you ask the girl to leave early, unless you get a chick with a half hour rate which isnt common in canada.
