This happened to me on Arid, but doesn't seem like anything map dependant to me.
I had an active shield pack when I went to get a repair pack from the emergency station, and when I picked it up, the active shield's sound kept playing. The visual effect wasn't there, only the sound. It would keep playing no matter what I do, be it shooting, skiing, jetting or using other packs, until I bought a shield pack, used it and let the active effect end by itself.
Pretty easy to repro, just use the emergency station with an active pack (any kind).

also my jet pack sound stays on sometimes :[[[
all of these are holdover bugs from ut2k3, which had some of the most funamentally broken sound code I've seen.

Somone hire this guy to be a bug finder, hes found more bugs during beta/demo/retail than most of the comunity together.

edit: btw I think the jetpack issue is related to lag
